D1000 / VxVm NODEVICE

Hi,

Have looked thru the archives and found a few similar problems but no
answers.

We had a disk go down on a D1000 so replaced it and after a few seconds the
green led was back on the array.

However we were still getting NDEVICE next to the disk, as we had planned
down time we bounced the server and array's. Re-allocation was not succesful
we got

Relocation was not successful for subdisks on disk oradg18 in volume u06 in
disk group oradg. No replacement was made and the disk is still unusable.

The disk as far as we can tell is fine, we then found out another disk
within the volume was reporting NDEVICE but this too appears to be fine.

Are we missing something?
